CRB personnel seem to delight in returning your forms for the smallest and most petty reason.

So how to avoid those errors!  Scrutinise their forms carefully when they bring them to you.

Is it in BLACK INK, CAPITAL LETTERS and within the BOXES.  NO PHOTOCOPIES!

SECTION A

Line 3 Make sure MIDDLE NAME is included (Check against Passport)

Line 8/9 Have they put their POSTCODE in. If they have been at their current address less than five years then fill in Section D. ADDRESS HISTORY IS A REGULAR ERROR

Line 10. DATE OF BIRTH crucial (Check against Passport or Birth Cert) Yes they do get it wrong!!

Line 12. NATIONAL INSURANCE NUMBER is crucial. (On Tax Form)

SECTION B

Line 13 They should fill in that they are Level 1 coach for U 10’s whatever

Lines 14 -19. You (CSO) fill in (Keep details of Club address and post code in your file)

SECTION C

Lines 20 -27  Married women have to fill in (Funny there are very few mistakes in this section!!)

Lines 28 – 35 DON’T LEAVE SPACES IN PHONE NUMBER

SECTION D

Need address history for previous five years. Use continuation sheets if more than once.

SECTIONS E, F & G –leave blank

SECTION H

Line 66 – applies to spent convictions. Line 67 – they can put them in a sealed envelope

Line 68 & 69 Sign inside box Line 70 DON’T FORGET DATE

SECTION X

Lines 1 – 4 CSO fill in from Passport          Lines 5 – 9 CSO fill in from Driving Licence

Lines 10 – 14 CSO fill in         Line 15 CROSS in box          Line 16 CSO  NAME IN BOXES

SECTION Y 

Personally I would specify the three items to bring to confirm their details – Passport, Driving License and Utility bill? (To check address or P54/60 to check address and NI No.)
